Culture : Ask for a culture to rule out a bacterial intertrigo secondary to strep or another bacteria. See dermatologist or urologist. If indicated, they can try treating with ciclopirox 0.77% cream and a low potency topical steroid cream for now. Can start an oral antibiotic if culture is positive. Consider a skin biopsy to rule out paget's disease, bowen's disease or another condition if not improving.

Get another opinion: If several doctors couldn't help you even with an in-person examination, you're not going to find help on the internet by someone thousands of miles away who can't see you.
