GYN exam: Please get a pelvic exam and evaluation. This can be caused by masturbation, inflammation, manipulation, tight thong underwear, aggressive sexual intercourse and multiple other factors. See your GYN, abstain sexually and get checked for urinary infection or urethritis

Here are some...: Without a direct look at, it is hard to offer accurate diagnosis. But all what you described could be just related with prominent clitoris & its hood like long foreskin in some men, leading to smegma and local irritation / inflammation. So, make sure to clean the area regularly and gently with compatible soap and see for 1-2 weeks. If failing to improve, seek evaluation & counseling. Best wish...
