See remark: There is one solution: he needs circumcision. In the meantime while in warm bath tub try gently retracting the foreskin little at a time,do it daily until you see a doctor,preferably a urologist

It happens sometimes: Boy's normally have a tight foreskin when they are younger. Parents teach their boys how to wash a penis, from about age 5 years onward. The pediatrician can check the penis every year at the check-up. If in late puberty, the foreskin is still tight, the pediatrician can help. Sometimes a cortisone cream such as triamcinolone is used. Circumcision is the last resort, as that is permanent removal.
