More data: A large red sore in the gluteal cleft can range from local skin irritation, infected oil gland [similar to a zit] to a the more serious pilonidal cyst. Pilonidal cyst form at the base of the back and the top of the cleft between the buttocks. Acute infection of a pilonidal cyst needs to be evaluated by a surgeon. Some may require lance local drainage and antibiotic coverage. More data please.
