Rule out phimosis: Your son may have a tight foreskin which can get swollen & inflamed when the tries to retract it. Also have some accumulated smegma (collection of dead skin cells like "dandruff" of penis) under foreskin which can cause irritation, swelling & even inflammation. Tight foreskin can be treated with steroid cream, requiring prescription, or circumcision. In mean time, antibiotic ointment may help.
