Dermatologist: If they don't bother you, forget about it. They can be individually teased out. The smaller they are the more tedious it is. No sutures or dressings. Small scabs heal in a week. But they need to be done one at a time so it can be a long process.

Best to see: a dermatologist, so as not to overlook anything important-what you think is "milia" may not be what it is ! They could be tiny little moles or freckles , contact dermatitis / eczema or a chronic fungal infection of the scrotum!
