Possibly: Though it sounds like a superficial skin infection called balanitis. Have him seen by his physician, and most antibiotics will clear it up.

Not necessarily: Sometimes a circumcised kid will just have irritation from rubbing against a diaper. Sometimes local staph skin germs get into the outlet & cause irritation. Sometimes it can be reaction to the soap he bathes in. His doc can sort it out.
