B-friend's penis has a couple red scabs. neosporin helps,but when penis gets hard,skin expands, the skin cracks.looks like it is scarred. thoughts?

Change creams: I cannot say if it is scarred or not but I would apply Vaseline or Aquaphor several times a day, and refrain from sex and masturbation until it heals-which will probably be a week. Any trauma to the area will prevent if from healing.
