Tubal ligation: fails roughly 1/100 to 1/200 times, depending on what method of ligation is used. Nor reason to worry: if you are late on a period do a pregnancy test today. If positive you need to call your doctor (or get one) ASAP, if negative you can wait a few weeks to see if/when the next period starts.
