Need biopsy: The base of these needs to be scraped and examined under the microscope for cells with inclusion bodies. Hand foot and mouth disease usually has a maculopapular rash caused by intestinal viruses of the picornaviridae family. commonly Coxsackievirus A16 and Enterovirus 71 but these are seldom painful. See a dermatologist. Best wishes.
