Placental abruption?: Abdominal pain and vaginal bleeding is placental abruption until proven otherwise. An ultrasound could be done to look for hematoma but if the fetal heart tones are concerning this this woman should be delivered quickly. If fetal heart testing is reassuring, the woman is stable, and bleeding is minimal, then a pelvic exam to evaluate the vagina and cervix could be completed.
