Many things: If you're still having periods and are sexually active, you need to take a pregnancy test to rule that out first. A normal pregnancy, ectopic pregnancy, etc can present this way. Once that is ruled out, the list is fairly long and will depend on your age, medical history, etc. Definitely something to see your doc about right away.

Cramps: You may be having what is called mittelschmerz - pain and cramping that occurs in the middle of the menstrual cycle - usually related to ovulation. It is generally a benign event except for the discomfort. Generally an over the counter pain medication like ibruprofen will help.
