Rarely: Rarely testicular atrophy (shrinkage ) is possible after complex inguinal hernia repair, if blood supply is cut off by constricting the cord that carries blood supply, especially older patients. It is a known complication of hernia repair.

Rarely: A critical step in inguinal hernia surgery involves dissecting around the spermatic cord which contains the blood supply to the testicle. A rare complication of hernia surgery is trauma to this blood supply, which can lead to infarction of the testicle. This will manifest itself as acute enlargement of the testicle followed by gradual shrinkage. This is more common in re-do hernia surgery.
