Ossicle?: You may have an ossicle small extra bone under the joint. X-ray will determine the cause.

Toe Pain: There is an arthritic condition of the big toe joint that causes the big toe to not function properly and therefore the big toe presses harder down into the shoe. The pain that you experience is one indicator for this form of arthritis and if you notice whether there is a callous located near the site of your pain i would be more convinced that there is arthritis at your first toe joint of your foot.

Big toe: It is unlikely to arthritis at your age. Not impossible, just unlikely. You may have hallux extensus - active , that is only when the tendons fire - or what i call continuous - the toe is in that position all the time. See your podiatrist for x rays - that will show the sesamoid if it is there as well as any DJD as well as the position of the toe.
