Is chest discomfort during exertion always cardiac related? what else might cause discomfort during exertion? anything besides muscular scelator?

No: But my first concern will be to r/o chd, specially if two or more risk factors present. Evaluation with ekg and stress test indicated. Another possibility is reactive airway disease/asthma and possibly gerd causing esophageal irritation/spasm. 50 millions american suffer from nsaid's induced gastric/esophageal irritation. Methacholine challenge test and endocopy possibly indicated.
