Not always: Typical symptom is chest pain however heart attacks could present with shortness of breath, fatigue, weakness, etc. About a fourth or more of heart attacks are estimated to be silent, i.e. Without symptoms.

Not always: Some folks actually do not have pain n the normal sense. Diabetics are often in this group.
