Ecg: Q waves mean either electrical force going away from the lead axis being examined or lack of force in the direction of the lead. This is often due to heart muscle damage but not always.

No: Some Q waves are the result of normal electrical activity in the septum (the muscle between the left and right ventricles). Misplaced ECG leads can also be associated with q waves. Q waves not associated with heart attack can be found in patients with copd, chest deformities, hypertrophic cardiomyopathy, and poor squeezing function of the heart (among other reasons).
