Ice & CLEAN scissors: You can always have your doctor remove them but if not, you can apply ice to the skin tag to numb it up, then make sure the area is cleaned off with an alcohol swab and with a sharp pair of very clean scissors (also wash with alcohol swabs), cut off the tag. Put pressure on the area to stop the bleeding and cover with a bandaid and some polysporin (bacitracin and polymyxin).

Physician removal: Stop Aspirin before removing skin tags. A physician should use sterile technique to remove skin tags and place antibiotic ointment on these wounds until healed. Some burn them off, e.g. Laser or bovie, some use a scalpel. These skin tags result from friction on the skin, and one can decrease the occurrence by putting aquaphor or vitamin e on the skin to decrease abrasion or from mechanical rubbing.
