Well, not really: There are certain autoimmune disorders that affect the early placenta that couse recurrent pregnancy loss but the reason for the majority of early miscarriages are chromosome issues.

Yes and No: Placental insufficiency usually relates to the third trimester of pregnancy. A placenta that doesn't work well could cause a miscarriage but it would not be detectable.
