See your doctor.: This constellation of symptoms should be evaluated by your doctor. The rib pain and shortness of breath could be a pulled rib muscle, but there are other possibilities. Your ear needs to be examined to rule out an infection or foreign body. I can't even help with the white spots, without more information. Let your doctor do a proper examination.
