Must be seen by a: Physician to make sure he did not have torsion of testis , or even a testicular tumor , . If you don't have insurance go to a city hospital

Might need surgery: If acute severe pain, vomiting, bloating then go to ed, could be an emergency. Otherwise look for free clinic or other subsidized care, lumps could be tumors, hernias or other conditions that need tests and or surgery.
