Go to mayo clinic : Complex issues do best with coordinated evaluation and treatment. I've had a number of patients go to mayo ( minnesota and florida at least) and may be worth the time and expense. Chronic illness in a spouse is an incredible challenge though so don't feel unusual. Consider couples counseling too. Good luck!

Bigger issues: Gout, pe are diseases for which prevention is key. Cervical discs may cause pain where pain reduction may only be partially possible. Question is: are goals unrealistic? Zero pain rarely achievable. Or is there secondary untreated depression? Or this is a "poor me, why does this all happen to me" issue. Declining social life, sex, work life requires action. New doc maybe? New partner?
