Sciatica: Depends on the cause. If it is due to a disk problem epidural steroid injections are worth trying - if no improvement surgery is always an option. If it is due to sacroiliac joint inflamation then injecting the joint will relieve it in 60 % of the cases. Occasionally I have to perform radiofrequency ablation for longer term relief.

Depends: I'd recomend being evaluated as there are numerous treatments that are available for radicular and sciatic type nerve pain including nsaids, oral steroids, injections, pt, surgical decompression and all of there usefulness depends on your particular situation.

Pinched Nerve: Sounds like you have a pinched nerve in your back. I would suggest seeing a pain/spine specialist to evaluate you further to evaluate what level and what can be done to treat you. There are interventional treatments besides medications and surgeries that might reduce or eliminate the pain altogether.
