Get checked out: New headaches that are very different from your typical ones, those with mental status changes, or with other neurological deficits warrant emergent new imaging to rule out things like a hemorrhage.

?new symptom?: If this is a new symptom, you should phone your doctor's office or consider going to urgent care. That said, your med list looks like you are on treatment - consider old school abortives like naproxen, caffeine, or a triptan if you have them and your doctor has suggested them as safe. In an er, other abortives (like dhe-45) can be administered.

See a neurologist: You should see a neurologist and have further testing to include a MRI and mra of the brain. You may need other testing as well. Some neurologists are experts in treating headache and have done fellowships in this.

Try OTC meds: You can try over the counter meds or call your doctor for advice. Migraines are never fun, but there are many good treatments available. Feel better.
