Carpal tunnel @night: On the microscopic level, cts is caused by inadequate blood supply to the median nerve. Typically when we sleep at night our blood pressure goes down and there is less nerve perfusion (blood supply). Also, sleeping on one's side with the hands tucked for minutes or hours at a time further compresses the nerve and its blood supply. This is why night splinting is so effective early on.
