48 hours-72 hours: Having 4 teeth removed that leave big holes will create an inflammatory response to promote healing. Ice helps as does homeopathic arnica and hypericum...One patient i had had remarkable decrease in puffiness in 8 hours after taking hypericum. Usually the puffiness is better in 2-3 days.

2-3 days: Usually after any surgical procedure, maximum swelling (puffy cheeks) occur 3 days afterwards. After 3 days, the swelling can resolve quickly. However, there are different ways to reduce the amount of swelling after the wisdom teeth removal: ice packs to the face, short term steroid pills or use of herbal supplements.
