Just ask: If you are taking pain meds, it's a pain management discussion. Topics can be "how to use less, " "preparing for surgery, " "referral for pain procedures, " "inadequate control, " etc. If they write the script, they'll want to discuss any changes. If you have pain, they will work it up and treat it. Best to just be upfront and ask.

Details: If you are having pain issues, make sure your doctor know all the symptoms you are feeling. That way the real diagnosis can be figured out and the best treatment plan placed. They can institute some conservative interventions, and if they don't work, then you can ask to seek a specialist for further care options.

Raise the topic: You may need to initiate it. I frequently hear patients say that nobody asked about their pain. While that is not good, you need to take charge and ask the questions.

Come right out!: If you have a pain issue, just come right out with it. A good history, with emphasis on exact nature of the pain, coupled with a good physical exam, can reveal a lot about the cause of the pain and how to treat it.
