Germs, injury & OA: Germs, eg bacterial, specifically sexually transmitted diseases/infections cause pelvic inflammatory disease (pid). Muscle spasm in back is often due to injury. However, there are many other causes of back pain. Osteoarthritis is most common cause of hip pain but likewise, there are other causes, too. Best to go see your doctor for a complete evaluation.

May not be related: The muscle issue could be related but not the pelvic inflammatory specifically disease. You need a specific diagnosis to help direct your physician's therapy.
