Might be serious: If this is new chest pain, it could be cardiac, meaning coming from your heart. This requires immediate medical attention. If you know it isn't your heart for absolutely sure, other things can cause the patterns of pain you describe - radiating pain from your stomach or pancreas, bad gas, or a muscle strain are a few of the possibilities.

Shoulder and chest: Pain there other than pleurisy may include cardiac, referred gallbladder, muscle strain, sprain, referred radicular pain.
