Be patient: The cream should kill the scabies within the first day of treatment. However, the skin disruption is caused by a reaction to the mites burrowing under the skin and leaving its debris in the tunnels. It can take two or more weeks for the skin to settle down and begin healing after initial treatment. If concerned a second treatment after a week could catch any stragglers, but is probably unnecessary.
