May be nerve issue: Intense itching can sometimes indicate nerve or blood vessel injury. First make sure shoes are long enough and tight enough across arch to keep middle toe from hitting end of shoe when you walk or run. You might also try a metatarsal pad which sightly elevates and spreads apart the metatarsals and takes pressure off nerves and blood vessels in ball of foot area.
