HIV: Petichiae occur due to either capillary fragility or to some defect in coagulation, usually platelet deficiency. The latter may occur due to a wide number of mechanisms, many of which can be seen in either acute HIV disease (splenic enlargement or bone marrow disease), or due to any of the infections or malignancies that following profound immune deficiency due to hiv/aids.
