Lots of potential: Areas. The most common would be nose or gums, but could also bleed internally in the gut, bladder or lungs. The most serious site would be in the barin.

The most ominous....: Site of bleed is the head. Bleeds elevate intracranial pressure. Because the brain is contained in a fixed space, its only place to go is out holes at the skull base. This is called herniation, and can lead to death. Notably, increased icp can slow/stop bleeding (tamponade).. Also, end stage pts are often refractory to PLT transfusion. Mucosal/internal bleeding can happen any time plts low also..
