Knee Cap: Swelling around or above the knee cap can be from the joint itself. This can be caused by inflammation, infection, metal allergies, or loosening of the prosthetic. If outside the knee joint at the knee cap can be bursal swelling, tear of quad tendon with hematoma. Would follow up with your surgeon that did the knee repalcement if possible.

Not from RLS: Your symptoms are complex and need interview and exam but seems not related to rls. The nature of RLS is progressive so discuss with treating physician to consider optimizing your treatment plan.
