Rot cuff surg jan back to work june pain returns 3 weeks later. mri says repair healed well but lots of swelling and inflam. cort shot no help. why?

Steroids are: Anti-inflammatory medication , hence will interfere with healing process . Most likely you may benefit from local treatment like heat and mild pain medication

Tendinosis: Tendinosis is a disease of the tendon which is not considered inflamed. Therefore cortisone would not be effective as it is an anti-inflammatory. Choices include further strengthening exercises to remodel the tendon. Currently seeing the use of regenerative injection therapy such as platelet rich palsma. This is somewhat controversial and the jury is still out about long term results.
