Dvt: The only lab test that may be helpful is a d dimer test. If it is negative it is unlikely to have dvt. If positive it is not specific for dvt.

Maybe D-Dimer: In addition, using a clinical tool like the wells criteria can help determine the likelihood of a DVT given various factors.

DVT: D-dimer can be used, but it is not diagnostic. Ultrasound is definitely recommended. In new onset dvt/pe it is also reasonable to check labs for hypercoagulability, including factor 5 leiden, among many others.

DD: I would disagree with Dr Brauer. High sensitivity Dd assays have high NPV and can safely R/O disease in low risk patients in conjunction with a validated VTE clinical prediction rule such as the Wells prediction rule. Thrombophilia testing, especially in the acute phase of VTE, has no role except age and gender appropriate cancer screening and possible anti phospholipid syndrome

Dx of dvt: order D- dimer, to see if it is elevated. An elevated level suggests thrombosis
