Several issues: If the influenza is so severe that they cannot hold anything down and need intravenous fluids, or if they develop primary influenza pneumonia, or if they have secondary bacterial complications (usually pneumonia), or if they have underlying illnesses which are made worse by the influenza they may need hospitalization.
