Complicated but: Imaging indicated in sinusitis for chronic infections (>90 days), recurrent infections, concern for complications of sinusitis (e.g. Periorbital cellulitis, brain abscess), or preoperatively before sinus surgery. Ct scans are almost always done, but mris are helpful for intracranial or orbital extension. Threshold to image is higher in pediatric population in order to avoid unnecessary radiation.
