What systemic blood system disorder could present with labs showing low wbc, neutropenia and lg spleen it normalizes and a month later is low again?

Many causes: It can be viral such as ebv. You may have low hem atomic levels such as low b-12. The concern is to rule out more serious and complex disorders such as primary bone marrow or lymphoid disease you need to be seen and have work up done. Ask for a hematology referral.
