Could be a cyst: Cysts are not uncommon behind the ear. They can get inflammed or infected and cause pain. If it is becoming infected, it will likely turn red and may drain pus and relieve the pain. If not , it may need surgical drainage or removal. If it continues to be painful, see your physician.
