Long second toe: Very commonly, if you have an elongated 2nd toe, the nail can be thickened due to pressure against the end of the shoe. If can also cause contracture of the toe and hardening of the skin at the tip of the toe.

Could be: While i agree with dr. Teppers answer it could also be the beginnings of an ingrown nail. Press on flesh on both sides od nail folds with pressure and see it that causes pain too.
