Two placentas: Not quite sure what's going on based on your description. But it may be she started out with two placentas and two babies, but it happens sometimes that one of the babies dies while the other one continues to live, but two placentas may remain.

Anatomy: It's possible that your wife had a twin that passed away. It's also possible that placenta is one placenta but with a separation in its membrane.
