Many causes: Swelling in lower extremities has a number of causes. Some are local things, some are systemic causes such as worsening congestive heart failure. Local causes certainly include blood clots, but also infection, local edema (such as dependent edema after long travel), muscular injury. If this is acute and never has happened before, you need to get it worked up to know cause.
