Can't really tell: You can't really tell for sure without an x-ray. Some sprains can be so painful that it mimics a fracture. However, if you have a fracture and it is not fixed properly, it can lead to permanent dysfunction of the joint or nerve injury (e.g. Pain).

You need X-Ray: If you have injured your hand or finger, you really can't tell a sprain from a fracture just by looking at it. The only way you can be sure is with an x-ray. I really don't like it when someone comes to me 2 months after their 'sprained finger' only to find that the finger was broken. The treatment for a sprain if very different from a fracture. Get it checked out.
