OK unless recurring: Even if it is blood, a small amount, one event would not be anything to worry about. It could be due to the thermometer causing some mild trauma or constipation. It could also not even be blood. I would have it checked if it is recurring or if there is a larger amount of blood.

Red spot: Not sure why you checked your baby's rectal temp. It's not very comfortable to do so and you may have incidentally injured him, so that's the reason for the blood you saw. Constipation ? Tell us what his stool frequency is to determine if he is indeed constipated.
