Have no fever, no tenderness over maxillary sinus, but air fluid level showing and headache on and off. are antibiotics needed?

Sinus fluid: If you're fine, i wouldn't push anything. If you are asymptomatic, i don't think you have an infection. If you become symptomatic, then get treatment, which would include antibiotics and possible surgery if the antibiotics don't work, but i doubt that this will occur.

Follow your doctors: You have had work up doneby your dotor who knows your history and physical exam and imaging report.Whether to use antibiotics depend whether it is acute sinusitis or chronic or recurrent sinusitis, whether you have fever, if you also suffer from asthma and allergies, if you are febrile, if you have purulent nasal discharge.Discuss with your doctor and follow his/her advice.
