Metarsalgia: It may be metatarsalgia, or ligament damage to the joint capsules for your MTP joints 2-4. It could be from repetitive micro trauma (not necessarily a big injury) but over time due to overloading conditions, cause swelling and pain to the ball of the foot. You may need to be evaluated for ligament rupture.

Possible neuroma: The symptoms you describe are consistent with a neuroma, which is a benign nerve tumor affecting the nerves going to the toes. The pain is worse while walking barefoot on a hard surface or when enclosed in shoes. Steroid injections and/or surgery are the treatments available.
