The Joy of Sobriety: Thank you for being honest about your drinking. People who combine pain medications and alcohol die. I hope you are smart enough to realize this. It is surely in the news a lot lately. If you were in my office today i would do everything i could to compassionately get you into a detox medical setting and to work with you to treat the pain and the addiction.

Tough spot: Obviously avoid any NSAID medications. You would want to avoid any narcotics due to abuse potential and respiratory issues and you are already on gabapentin. You could try antidepressants such as Cymbalta (duloxetine) or effexor; both have excellent histories with helping pain. Tramadol for daytime pain, trazodone for nighttime pain. Also, really consider topical compounding creams containing ketamine.
