Heart attack: Real heart attack may be silent or with symptoms. The ekg may not show it, called non-st- elevation. However the cardiac enzymes troponin & CPK would be positive. Symptom wise- yes; test wise-no.

An ECG can be : Nondiagnotic or negative during a heart atrack. Blood test are usually proteins released to blood stream when the heart muscle get ischemic or die. Troponins and ck-mb. False negatives are very rare with the blood test. Stress test can be negative but sensitivity and specificity can improved with the type use. Cardiac cath will be positive if a unstable plague at coronary occur.
