Can a blood clot be lost on 4th day after tooth extraction?

Yes: Follow instructions given after the procedure. A dry socket is extremely painful if the clot is lost.

Not likely: The blood clot begins to organize into normal tissue. By the forth day it should be fairly stable unless you have done something to it to cause it to dislodge.
